Keratoconus | Diagnostics | Treatment | 1. Corneal Cross Linking (CXL) | 2. Implantation of intrastromal segments (rings) | 3. Laser correction by corneal topographic finding | 4. Corneal Transplantations | Corneal degenerations and dystrophies | Infectious diseases and corneal injuries

Keratoconus - cornea centre

Cornea is the clear and transparent texture through which the light rays enter as the first the eye. It is the gateway to the eye and when the cornea is damaged, the light may deform differently and in some cases very weakly or even not at all.

Keratoconus

Keratoconus belongs to the group of ectatic corneal diseases. It occurs in one in every 1000 to 1500 people. The cause of this disease is not fully known but it can be assumed that it is an combination of several factors: heredity, mechanical influences or allergy.

The first symptoms of keratoconus occur in the second decade of life (between 10th and 20th year of age). This disease is predominant in young men. Diagnosis is often linked to other diseases, e.g. atopy, Marfan's Syndrome, Down's Syndrome, Turner's Syndrome.

The development of the disease is linked to increased myopia (negative dioptres) and astigmatism (cylindrical dioptres). However, these cannot really be corrected by using spectacles.

In the past, it was only possible to influence the development of the disease by using hard contact lenses. Today´s treatment options are much wider and their suitability depends on the stage of keratoconus.

Diagnostics

Diagnosis plays a key role in detecting keratoconus and its classification into individual stages of progression, of which there are five: the fruste form of keratoconus and stages I to IV. Depending on the stage of development of the disease, we will select the most suitable therapeutic method.

We perform keratoconus diagnostics using several specialised examinations:

- corneal topography

- optical corneal topography using a Scheimpflug camera

- keratometry

- aberrometry

- biomicroscopy

Treatment

1. Corneal Cross Linking (CXL)

It is a therapy, which uses two methods: non-invasive (without abrasion of the corneal epithelium) and mini-invasive (with abrasion of the corneal epithelium). The choice of treatment depends on the results of a thorough examination, analysis of the anterior and posterior surfaces of the cornea, its curvature and its thickness. In the majority of cases, we prefer the method without abrasion, which is less painful for the patient and non-limiting in the post-operative period.

The aim of the CXL method is to stop the progress of ectatic corneal disease and not to compensate the cylindrical defect which results from progression of this illness.

Procedure

In both cases, the intervention is carried out under topical anaesthesia. In the case of the non-abrasive method, this is mainly used for greater comfort. We apply a riboflavin solution and dextran to the patient's cornea at two minute intervals until we can biomicroscopically see the penetration of the solution into the anterior chamber. In transepithelial application, this phenomenon is usually observed after a 45 minute application of the solution.

UV-A exposure lasts 30 minutes with the continuous application of the riboflavin solution and topical anaesthetic.

After completing irradiation, in the case of the abrasion method a contact lens is placed onto the eye and covers the eye for the following four days. While the lens is in the eye, the patient receiving home treatment will apply antibiotic drops into the eye five times per day. After removal of the lens from the eye, therapy will move on to local corticoids similarly to transepithelial treatment. Corticoids are applied into the eye for three months. Subsequently, the patient attends regular checkups for six months.

2. Implantation of intrastromal segments (rings)

This is an invasive technique where one or two segments are inserted into approximately 80% of the thickness of the cornea after creating an intrastromal channel. The exact number depends on how large and spread the area of keratoconus is.

This ambulant surgery is preceded by a detailed examination of the cornea and the determination of the number of implants and the place of incision.

Procedure

The surgery itself takes approximately 25 minutes. At the end of the surgery, the eye is covered with a contact lens which will be removed on the fourth day after the surgery. Post-operative treatment is very short and only lasts seven days. As with CXL, the subsequent monitoring period is six months.

3. Laser correction by corneal topographic finding

In certain forms of early detections of keratoconus it is possible to perform two procedures in one: the treatment of the cornea with the excimer laser, which achieves a correction of the refractive error of the cornea and after that the CXL procedure is carried out (see above). That stabilizes the cornea and stops thereby the progress of keratoconus. This surgery is not suitable for everyone. Restrictions are given by the stage of the keratoconus, the extent of the refractive error and especially the thickness of the cornea.

4. Corneal Transplantations

We performed the first corneal endothelial transplantation in September 2008. Since then, we have also gradually introduced perforation and anterior lamellar transplantation of the cornea into our ambulant surgery.

Depending on findings and the level of damaged part of the cornea, we select the most suitable method, so the eye is burdened least possible and only that part is transplanted, which is really damaged. Therefore, we always consider transplantation through the entire thickness (perforating or only partial), i.e. lamellar, which puts the least burden on the patient's eyes.

During the first days after the surgery, we monitor the patient in accordance with a scheme stated in advance. Subsequent regular care is carried out by a local ophthalmologist as agreed with us in advance.

So far, we have carried out all transplants under local anaesthetic but, if necessary, surgery may also be carried out under general anaesthesia in our clinic.

We get grafts from the Eye Tissue Bank at the Ophthalmology Clinic of the University Hospital of Královské Vinohrady, Prague, which, in terms of quality, meet the highest requirements of ophthalmological surgeons around the world. In most cases, the waiting period for a transplant is shorter than eight weeks.

Corneal degenerations and dystrophies

This is a very broad group of diseases that more or less affect the quality of visual acuity and that are often accompanied by a distinct pain, photophobia, and lacrimation.

We consider the treatment depending on which part of the corneal is affected by the disease. Thanks to the complex facilities of VESELY |Eye Clinic we accurately identify the type of the disease and accordingly determine the proper treatment.

In the treatment we use all available therapies, the excimer laser, femtosecond laser, CXL therapy, application of amniotic membrane and transplantation to the cornea.

We performed the first corneal endothelial transplantation in September 2008. Since then, we have also gradually introduced perforation and anterior lamellar transplantation of the cornea into our ambulant surgery.

Currently we are the non-state institution with the largest number of transplants in Slovakia and the largest number of the so called posterior lamellar transplants at all.

Not only patients from all over Slovakia, but also from abroad undergo the corneal transplantation at our clinic.

Infectious diseases and corneal injuries

Corneal injuries caused by accidents or an acute current infection require immediate intervention and treatment.

Our continuously learning doctors apply in the healing process the latest knowledge and practices in the treatment of complicated corneal irritation and pacients and many colleagues from all over Slovakia address our specialists with confidence.

Femtolaser surgery

This is the method with the fastest healing time. The majority of patients will return to their normal life within 12 hours. The principle of this intervention is in the cooperation of two lasers. The first one, which is the femtosecond laser creates a protective flap on the surface of the eye. An excimeric laser is applied underneath this flap and removes dioptres. The exactness and precision of this process will allow you to look at your surroundings without spectacles and without any pain already just a few minutes after the surgery.

Methods

iLASIK

Your nearsightness, farsightness or astigmatism (cylinders) will be, based on the so called digital map (WaveScan) of your refractive defect harmlessly and perfectly reduced. We will gain this map during an individualized preoperative examination. This data includes information about the detailed curvature of your cornea, its unevenness and the presence of refractive defects of lower and higher degree. Based on this data, we will carry out the treatment which will accurately correspond to the needs of your eye so after the treatment your vision will be of the best quality and sharpness.

Femto LASIK

After a detailed examination and obtaining information about the level of your dioptric defect, you will undergo a treatment which will reduce your dioptres. After this treatment you can enjoy your life without the need for further spectale correction.

LASEK

This method belongs to the group of surface refractive surgeries of the cornea. The laser beam strikes directly the surface of the cornea after temporary removal of its epithelium from its original location. After the completion of laser treatment, the epithelium will be placed back in its original location and will act as a bridge for the newly growing epithelium.

PRK

Also this method belongs to the group of so called surface surgeries, where the laser beam directly effects on the surface of the cornea either without any previous intervention by the surgeon - the 'No Touch' method - or after the previous mechanical removal of the corneal epithelium by the surgeon. The PRK method is mainly suitable for patients with a thin cornea, who suffer from keratoconus.

Examination

You will first undergo a complex initial examination with the aim of gaining as much individual data as possible not only about your eyes, but also about your habits, hobbies and activities. Based on these results, our specialists will recommend you the most suitable and safest solution and further procedure for you.

Before to examination in the Centre of laser and refractive surgeries of the VESELY I Eye Clinic, you must not wear contact lenses for at least two weeks, in order to avoid distorting the results of this very sensitive examination.

The examination itself takes approximately two hours and it also includes the measurement of the visual acuity, measurement of intraocular pressure, the control of the anterior segment of the eye (cornea, iris, lens) and the fundus examination. Along with this you will undergo several soecialized examinations to help us determine the exact level of your refractive defect. For this reason, we do not recommend to come to the appointment by your own car. The effects of this examination (worsened near vision, the inability to focus) will spontaneously disappear within 3-4 hours after the examination.

Treatment process

Surgical procedures are painless and usually take less than 15 minutes. they are carried out in local anesthesia, pacient is fully conscious. Depending on the type of the surgery, you may experience discomfort such as eye burning, photophobia or increased lacrimation. These symptoms are characteristic for PRK and LASEK and can last for about five days. Treatments FemtoLASIK and iLASIK are completely painless and allow you to return to your normal life already after 12 hours after the surgery.

Phakic lenses - from 1 380€

The existence of modern phakic lenses means the possibility of correction of high and very high corrective defects also in patients who, because of their high level of dioptric defects, were not able to get rid of their spectacles.

A phakic lens is an implant made of very soft and flexible material which is inserted into the eye via a very small incision (2.5mm). The entire treatment is pain free and does not take longer than five minutes. Immediately after the surgery, you will be able to see without the need for any further corrections.

The whole method may be described as wearing spectacles or inserting contact lenses with the difference that you do not need to remove them in the evening.

Types of phakic lenses:

In our clinic, we use Ophtec Artisan/ Artiflex and STAAR Surgical ICL phakic lenses. Our specialists have with the implantation of phakic lenses rich and long experience.

Refractive lens exchange - from 810€

After forty years of age, the majority of people start to experience presbyopia. This age-related condition is characterised by worsening near sight (reading) and working vision (working with a computer). In the past, the only alternative was multi-focal spectacles or using two different glasses. However, today you can undergo treatment which will provide you quality vision for all three distances with a so called refractive lens exchange (PRELEX - Presbyopic Lens Exchange).

Implants

At the VESELY I Eye Clinic, we pay great attention to the quality of specialistic care and the selection of the implants. For this reason, we offer you implants from the two highest-quality producers who are the best in the world, not only in production and quality but also in implant development.

All the implants we use in our clinic are made of soft, hydrophobic acrylate. They can be folded and therefore their implantation through minimal incisions is possible. The most modern design and the artificial intraocular lens material provide the greatest visual acuity and reduce the risk of a secondary cataract. That is why the selection of an implant is very important for overall long-term results and satisfaction of our patients.

For this reason, we do not recommend to come to the appointment by your own car. The effects of this examination (worsened near vision, the inability to focus) will spontaneously disappear within 3-4 hours after the examination.

Before the surgery

It is not necessary to undergo an internal pre-operative examination allowing the surgery to be carried out under local anaesthetic. A week before to surgery, it will be necessary to apply drops into the eye, which will be operated, as recommended by the doctor.

A week before the surgery we rocemmend you to take a preparation for the protection of the retina, that contains omega-3 fatty acids. You can get this preparation at our clinic at the end of your preoperative examination within the package "protection of the retina".

Treatment

Alike the cataract surgery, this treatment is pain free and lasts less than 15 minutes. You will undergo several specialised examinations which will help our specialists to calculate the exact implant that will be most suitable for you. Thanks to this implant you can enjoy your quality vision again without glasses already on the second day after the surgery.

Surgery

Your stay in the eye clinic on the day of the operation will last approximately two hours. During that time we will check your condition and exclude any acute eye diseases. Then, drops will be applied to the pupil of the eye that will be operated. Surgery is carried out under local anaesthetic and is pain free. The surgery itself takes an average of 5 to 10 minutes.

The surgery itself can be done in two different ways.

Phacoemulsification

In the classical procedure, the lens must be broken by an ultrasonic sonde after the surgeon performs several previous actions, such as entry cuts, capsulorexa- hole in the capsule of the lens and more.

Into the free space is then inserted an artificial intraocular lens that replaces the original lense.

FLACS- Femtosecond Laser Assisted Cataract Surgery

FLACS is a method, that is more modern, more harmless and much safer. That means a cataract surgery assisted by a femtosecond laser.

On VESELY | Eye clinic we use as the only ones in Slovakia the laser CATALYS, which was from the beginning designed specially for the cataract surgery, unlike other lasers.

Femtosecond laser can perform up to 60% of the steps of a cataract surgery. This laser divides the lens into several hundred small regular fragments that have no longer be crushed by ultrasound but they can just simply be removed. This makes the whole procedure harmless and safer.

In the case of implantation of so called multifocal or toric lenses in cataract surgery we normally use the navigation system VERION from the company Alcon. Thanks to this system we can accurately determine the optimal position of the lens in the eye, so the result of the procedure is as perfect as possible and your visual acuity as good as possible.

After the operation you will get an operational protocol and identification card of the lenses (implant). Approximately 20 minutes after the surgery you can leave the eye clinic accompanied by a close person. You will see the result of the cataract surgery immediately after the surgery.

Pooperačná liečba a obdobie

After the surgery and a short observation time our staff will recomend you the necessary post-operative care once again and release you into the home care. The term of the first post-operative appointment is always individual. During this time it is very important to follow the instructions of the postoperative care and in case of any uncertainity to consult with our doctors.

The post-operative treatment is not difficult. After the surgery it is important to apply post-operative drops into the eye for three weeks. We do not recommend you lifting and carrying heavy items (over 3 kg), nor using excessive force or strain yourself. It is not recommended that you visit a swimming pool, lake or sauna. Normal work activities are permitted as well as free and undemanding movement. These limitations should not exceed the duration of 3 weeks. After that you can return to your normal life and hobbies with even better vision than before the surgery.

All advices and instructions are for your use included inside the folders that you get from us at the end of the preoperative examination.

Cataract centre

Cataract is a disease which sooner or later will affect all of us. In a large extent it is related to the age and it makes our visual acuity worse. Today, you no longer have to wait until the cataract matures. Your quality of life is easily to maintain thanks to the operation, which belongs to the most frequently performed operations on the human body. By using the femtosecond laser cataract surgery has become even safer and the eye is less burdening than in the past. VESELY |Eye clinic is the contractual partner of all health insurance companies in Slovakia and therefore is the cataract surgery fully covered by your health insurance without any additional charge for the surgery.

Cataract surgery – already without any additional charge

Creation and symptoms

The creation of a cataract is related to a build up of by-products from the metabolism in the human lens. Within the eye, the lens is located behind the iris and a human can focus near and far thanks to the lens. The accumulation of metabolic by-products causes clouding and the "hardening" of the lens. In the initial stage, this worsens vision but can still be corrected by using spectacles. However, vision subsequently becomes more blurred, which causes a worsened sense of colour, contrasting environments, difficulties with driving in dusk and at night and many other symptoms.

Treatment

The only way to remove a cataract is surgery. Eye drops, ointments or various eye exercises are ineffective. This surgery is not challenging for the patient. The patient does not feel or sense the operation.

The range of implants is very wide and includes mono-focal lenses and lenses with a so-called 'yellow filter', toric lenses, as well as multi-focal lenses which are included in the group of 'premium implants'. We always try to adapt the implant selection to the age, diagnosis and requirements of the client. We would be happy to advise you and recommend the most suitable implant for you.

Examination

Before cataract surgery in our eye clinic, you will undergo a complete examination of both eyes. This includes checking visual acuity, the level of intraocular pressure and a funduscopic examination. You will also undergo several specialised examinations which will help us to calculate the exact value of the implant most suitable for you. Based on the obtained data, we will order the implant for you.

Post-operative treatment

After the surgery it is important to apply post-operative drops into the eye for three weeks. We do not recommend you lifting and carrying heavy items (over 3 kg), nor using excessive force or strain yourself. It is not recommended that you visit a swimming pool, lake or sauna. Normal work activities are permitted as well as free and undemanding movement. These limitations should not exceed the duration of 3 weeks. After that you can return to your normal life and hobbies with even better vision than before the surgery.

All advices and instructions are for your use included inside the folders that you get from us at the end of the preoperative examination.

Health in hands of professionals

We are pleased that our work helps to improve lives of many other people. Satisfied patients are our greatest reward.

I came, I saw and now I see even better! I started this new "sharp" life without lenses and glasses in 2012 at VESELY Eye Clinic. I will always be grateful to all the staff of the clinic led by doctor Veselý, for the smooth process, excellent approach and fantastic result. Thank you

When I started squinting, I said to myself: it must be the pressure :) When I started having problems to focus, I said to myself: it must be the age :) When I started having problems with face perception, I said to myself: it must be my bad mood :) But when I was not longer able to see the world around me, I was growing sad :) Now I am happy to be able to see the world again, such as it really is! It doesn't matter whether it's good or bad! You shouldn't blame the mood or the age or the pressure! All you need to do is to visit the Vesely eye clinic, as I did, and the world around will look much brighter :) I am happy with my choice – thank you! :))))

I have had problems with my eyes since I was a student – they got inflamed quite often and I was going from one ophthalmologist to another. The situation got better over the last few years, as I became a patient at Vesely clinic and now I can contact them every time I have any new problem with my eyes. Since I have been in their care, I have developed more responsible attitude and I take better care of my eyes. I am very satisfied with the care provided. I feel good at the clinic and I am happy to be one of their clients. I trust both my doctors and would like to thank them for their helpfulness, professional care and friendly approach. Thanks to them I am less worried about my eyes... and it feels good...

When driving on unknown roads I realised that not only my near vision without glasses was zero, but my distance vision was getting very poor too. As it was getting dark, I had big problems to read the road signs. So I decided to deal with this handicap. I received very good recommendations about Vesely doctors from my friends and family. I called VESELY eye clinic and made an appointment. After the check up doctor Vesely suggested an intervention called PRELEX. After a short consideration I decided to undergo this intervention and now my near and distance vision is perfect even without the glasses. I am very satisfied with my decision as well as with the work of doctor Vesely and I thank him for this new life without glasses.

It's still a miracle to me... I was wearing glasses with high diopters since my childhood. I hated them, they were hindering me and I often felt ashamed for them and didn´t wear them. Later I started using contact lenses which were more practical, but irritated my eyes and so I could not wear them as often as I wanted. Doctors were always saying, that there is no solution or surgery suitable for me. Until a year ago, I learned through a friend about you, your clinic and surgery, which would allow me to see well without glasses. After completing the initial examination and after surgery explanation of what it is, what are the conditions and costs, I decided to undergo the treatment. The intervention itself was really short and essentially painless. My vision was sharp right the next morning. I really saw everything, everywhere, even in the distance without glasses or lenses:-). It's still unusual for me, that I can see, just like that, without anything, even better than I've seen with glasses. Today I see buildings in the distance, hills and other things around me that I have not seen before, and even didn´t recognize them.
The most amazing feeling for me is, that I open my eyes in the morning and I can see. Thank you for doing what I already believed to be impossible.

I was very satisfied with the overall approach, including the preoperative counselling, consultations, series of examinations, the surgery itself and the postoperative care – it really exceeded my expectations. I appreciate the professional approach on one side and the perfect approach of all the staff led by doctor Veselý on the other side. The surgery went exactly as expected: it was painless and lasted just a few seconds. The convalescence period was extremely short – I was able to use my eyes without any limitations already one day after the surgery. If I was to decide about the surgery again (which I don't have to, of course), I would not hesitate.

I would like to thank the whole team of VESELY | OK Eye Clinic for the biggest and best change in my life, for the professional performance in a nice atmosphere, thanks to which I can finally see well. And I have a message for all those who still complicate their lives with glasses and lenses: the world without them is really even sharper and more colourful – any time you open your eyes!

After I decided to undergo the eye operation, I was pleasantly surprised, as in VESELY | OK things went quite quickly. Two weeks after the first check up, I already had the operation. On Friday evening after the surgery I was watching TV and on Monday I started to work, already forgetting that I ever used to have bad vision. I was happy to see everything. Some thirty-minute surgery is definitely worth it! I would like to praise the perfect doctor, nurses and their fantastic attitude, not only during the surgery, but also during the postoperative checkups. If I was to recommend a clinic for eye operation, I would definitely go for VESELY | OK.

I heartily recommend Vesely Eye Clinic to all the people who are considering getting rid of glasses or lenses. I was very pleased by their professional approach, modern equipment and agreeable environment. The surgery itself was very quick, painless and not followed by several days of burning eyes, so one could already start enjoying the life without glasses after a few hours.

Ronald Lupták

I had been considering laser surgery for several years, literally. However, I was discouraged from trying the usual method by a number of my friends, who went through it and described in detail the postoperative pain they had. Then I learnt about the new painless method. I visited Vesely OK Eye Clinic and finally made the decision. I had an examination at the clinic, we planned the date of the surgery and for a certain period I had to stop wearing lenses to get ready for the operation – so I had to go back to using glasses. I was very curious about the intervention, as I was not able to imagine it. The team of doctor Vesely prepared me very well – they provided me with enough information not just before the surgery, but also during the process. The interventions itself did not cause me any pain. The feeling was unpleasant, but that is logical, as interfering with one's eye will always feel unnatural. But it definitely was not painful, and that was very important for me. The evening after the surgery I already noticed that my vision is sharp, even if slightly foggy at first. I will never forget how I was looking from a balcony on the 5th floor and was able to read vehicle registration plates without any glasses or lenses! For me it was a miracle. The following day I even drove the car on the way to the first postoperative check up. Without glasses or lenses! The postoperative period was amazing. My foggy vision gradually cleared, I had no pain whatsoever and the second day after the surgery I was already able to deal with everyday situations. I was not extremely sensitive to sun, my vision was sharp and above all, I did not feel any pain. Besides, I rediscovered how it feels to see objects at large distances and until nowadays I have the impression that my vision is sharper than any time before. What I also value in VESELY | OK is the postoperative period. I used to go for regular checkups, during which the team of doctor Veselý monitored my convalescence and together we rejoiced at the result. I think that nobody, except for short-sighted people, cannot imagine how amazing it is when the first think in the morning is not that you take your glasses from the night table or apply the contact lenses into your just-awoken eyes. I do not need to remember to use the lenses instead of the dioptric glasses in case I am planning to wear sunglasses during the day. I can even dive or do any other sport which would be quite complicated with the glasses. Unbelievable. If I was to decide once more whether to go for the laser eye surgery or not, I could confidently say: definitely yes and definitely at VESELY | OK.

Common questions

What is the waiting time for a first examination?

For a first examination the waiting period can be up to 10 days.

Is is necessary to bring documents from previous examinations from other hospitals or clinics?

It is not necessary. However, if you suffer from a long-term eye problem, any piece of information is valuable.

Will it be necessary to dilate the pupil?

In the case of a pre-operation examination or a total eye examination it is probable to expect pupil dilation eye drops. This will in turn result in a very short where driving and certain actions (such as working with a computer) are affected.

What does a first examination consist of and how long is it?

It consists of a complete eye examination. It includes: the measurement of eye pressure; thickness of the cornea and the diopters; a control of the sharpness of your sight; examination of the anterior and posterior segment of the eye. These are then followed up by more specialised examinations in accordance with your particular disease. (Galilei - analysis of the cornea; Perimeter - measurement of your field of view; OCT - CT examination of the retina, cornea, iris and the angle of the vitreous humour. The length of a complete examination is usually one and a half to two hours.

Cataract surgery

Can I come for Cataract surgery without a recommendation from my doctor?

Yes, you can come without a recommendation from your doctor.

Is the operation covered by insurance?

Yes, Cataract surgery is covered by insurance.

Are both eyes operated at the same time?

No, there is a time period of one week between surgery of different eyes.

What is the waiting time for an operation?

On average, the waiting time for an operation is shorter that 6 weeks.

Is the procedure painful?

No, it isn’t. The operation is carried out under local anaestethic with the patient in full consciousness. Thanks to the local anaesthetic the patient does not feel any pain during the procedure or after it.

How long does the procedure take?

The procedure it self take about 7-10 minutes. Before it it is necessary to examine the eye and dilate the pupil. All together the patient spends about 2 hours at the clinic.

How long is the recovery period after a procedure?

During the procedure, only a minimal incision is made into the cornea, the recovery period is very short. To the vast majority of tasks you performed before the procedure, you can return to them the next day.

What restrictions am I expected to experience after the procedure?

It is recommended that on the day of the procedure the patient has calm routine. About 3 weeks after surgery you should avoid heavy physical exercise. You should not carry very heavy loads or pick up heavy things when you are bent forward. Furthermore it is not advised to, in those 3 weeks, to involve yourself in extreme or contact sports. Swimming in a pool, lake and the sea is completely prohibited during this time period.

Is it possible that my Cataract will return after the surgery?

No, it is not. Due to the fact that the human lens is removed during the procedure and a new replacement lens is implanted, a Cataract cannot reform.

What is the replacement lens made from?

The lenses we use in our procedures are made solely of hydrophobic acrylic, a special material, which shows the best properties and the smallest amount of unwanted reactions.

Is it possible that my body will reject this implanted lens?

In medicine, absolutely everything is possible. However we have never experienced something like this in any of our procedures.

How long after the operation do I remain in the care of doctors at VESELY | Eye Clinic?

You will remain in our care one week after the procedure, at the least. After this, your own eye doctor will take over your care. If you have not come to us with a recommendation from your eye doctor you remain in our care for the next 3 weeks.

Will I have to use any eye drops after the operation?

Yes, after the operation it is necessary to apply antibiotic eye drops up to (at a minimum) 2 weeks after the operation.

Laser surgery

Is this procedure covered by insurance?

Laser eye surgeries belong to the group of cosmetic procedure and therefore they are in general not covered by insurance. The procedure will be partially covered by insurance if the difference in the dioptrics between one eye and the other is more than 3.5 diopters.

How do I know which procedure is right for me?

After the careful first examination and after the obtaining of information about the severeness of your dioptric fault, along with other necessary information about your eyes and your sight, you will be offered the best method to remove your fault after the consultation with your doctor.

Is there an age limit for laser eye surgery?

It is possible to perform laser eye surgery for people in any age group. The only condition is a physiological find in the other structure of the eye. Due to this, customers who are older than 40 years old can in some cases also undertake this procedure. However we do not apply laser surgeries on customers younger than 18 years old, because of the uncompleted growing and developing of the organism.

Is the laser removal of dioptres limited by the level of dioptres?

The equipment in the VESELY Eye Clinic is able to remove refractive errors within a range from +5.0 dioptre to -12.0 dioptre, even in combination with astigmatism. However, in the majority of cases we do not correct extreme refractive errors using a laser since we do not consider such a solution as correct and safe. We regularly use laser correction for longsightedness up to +4.5 Dsph and for shortsightedness up to -7.5 dioptre. We consider the removal of greater refractive errors by laser to be safe and suitable only in cases meeting the anatomic conditions in the eye. We correct greater refractive errors using phakic lenses which, in these cases, we consider to be a more suitable, less invasive and safer solution.

Is it also possible to operate on a lazy eye?

It is also possible to correct a lazy eye using laser surgery and provide sharp vision without spectacles as it was with spectacles before the surgery. However, we are not able to remove a lazy eye disorder. The truth is that each of our clients with a lazy eye who undertook iLASIK treatment achieved better sharpness of vision without glasses than before the treatment when wearing spectacles.

Are both eyes operated on at the same time?

Yes, laser correction is carried out on both eyes at the same time.

Is this surgery painful?

No, laser surgery is carried out under local anaesthetic, thanks to which the treatment is totally painless. However, due to the treatment, in the post-surgery period you may encounter unpleasant sensations (a feeling of the eyes being cut, burning, photophobia or increased tearing) lasting for several days. These symptoms are characteristic for PRK and LASEK. If the treatment was carried out using a femtosecond laser - FemtoLASIK and iLASIK - the post-operative period is painless and you may return to your everyday life the next day.

How long does the operation take?

The stay in the clinic usually lasts two hours, during which time all the results are re-checked, drops with an anaesthetic effect are administered into the eyes and then the surgery is carried out. The laser treatment usually takes less than 15 minutes.

What is the difference between PRK or LASEK and LASIK treatment?

All three belong in the group of laser treatments for correcting refractive errors. PRK and LASEK treatments are surface treatments. This means that the laser beam affects the surface layers of the cornea - the epithelium or Bowman's membrane. Only after their removal does it become effective in the corneal stroma, i.e. the part which is more suitable for laser correction. The fact that this treatment causes irreversible removal of the Bowman's membrane, the corneal anatomy is completely changed. During PRK or LASEK treatments, there is irritation and damage to nerve endings in the cornea and therefore convalescence after these treatments is very painful and unpleasant. LASIK treatment carried out using a femtosecond laser is one of the sub-surface treatments. It is characterised by the fact that a fine and thin flap is created on the eye's surface. Under this flap, which contains the epithelium, the Bowman's membrane as well as the very fine part of the stroma, the removal of refractive errors is carried out using an excimer laser. Thanks to maintaining the integrity of the surface layers of the cornea, convalescence is painless and quick since there is no massive irritation of the nerve endings in the cornea.

How long is the convalescence period after the treatment?

The length of convalescence depends upon the type of treatment you select. a, for PRK and LASEK, the total period of convalescence is approximately six months. Early post-surgery convalescence is approximately 5 - 7 days. This period is related to a cutting and burning sensation in the eyes and pronounced photophobia and tearing; therefore, rest at home is recommended. During the whole period, covering contact lenses are placed in the eyes and are removed after five days. It is necessary to administer drops into the eyes on a daily basis from the day of surgery until the end of treatment. Treatment is finished six months after surgery. b, For FemtoLASIK or iLASIK, the total period of convalescence is circa two weeks. Early post-operative convalescence lasts less than 24 hours, after which time you may return to work, school or your hobbies. Post-operative treatment lasts just a week.

What limitations are expected after the surgery?

It again depends upon which treatment is decided upon. However, in every case, you should avoid swimming in pools, lakes or the sea for at least two weeks. It also recommended that you limit activities in the area of contact sports - boxing, judo, karate, basketball, handball, …. for the same length of time. You can return to extreme sports approximately four weeks after surgery. The basic difference is in the length of post-operative treatment which depends upon the type of surgery, as mentioned above. During the course of treatment, becoming pregnant is not recommended, i.e. for six months in the case of PRK or LASEK. This is due to the long term treatment, although with very mild corticosteroid eye drops. In case of FemtoLASIK or iLASIK, this issue does not apply since the treatment only lasts a week. However, the limitations may change individually depending upon the treating physician's instructions.

Is it possible that the errors with vision will return after laser surgery?

All examination methods point towards reaching the final correction that is most stable and permanent. However, in some cases, there is spontaneous tissue regeneration which cannot be influenced by the patient's will or efforts by the physician. The probability of the relapse of a refractive error after laser surgery is less than 0.1%. In such a case, a possible solution is free laser re-correction. Such a correction is provided within the "Satisfaction Guarantee" with VESELY Eye Clinic offers to every client completely free of charge.
